About Achieve Opportunity

Achieve Opportunity is our name for Community and Employment Support (CES) Waiver, often called "medicaid waiver" or "IDD waiver".

​

Achieve Opportunity provides 1:1 support in the client's home providing the support needed to maintain independence and build self-care skills.

​​

This program is offered through approval from the Department of Human Services (DHS). DHS takes applications year-round although there is a long wait list for approval of services. Once approved for CES Waiver, families can contact Achieve Community Alliance to get setup with a caregiver or you can choose to bring your own caregiver to Achieve Opportunity. If you choose to bring your own caregiver, they'll need to be hired through our typical HR hiring process including background checks and orientation. We would love to talk with you today about getting setup on Achieve Opportunity!
